Seek care quickly for a hot joint, major swelling, bad injury, or sudden weakness. Most soreness isn’t an emergency, but these changes are safer to check now instead of waiting through the night or another full day.

## When to get same-day care

A hot, swollen, very painful joint needs same-day care, especially if you feel chilled or ill. Gout from sharp crystals in the joint can look like infection, and fever may be missing.

Tell the care team about a joint replacement, recent surgery, diabetes, or nearby skin infection. Don’t rely on a home checklist to rule out an infected joint.

## When to seek emergency help

Get urgent help after a serious injury that bends the joint out of shape. Quick care also matters when you can’t stand on the limb or it turns pale.

Loss of bladder or bowel control with new leg weakness can involve the nerves. If you can’t travel safely, call emergency services or ask someone nearby to call.

## When to arrange a prompt doctor visit

Several swollen joints or long morning stiffness deserve a prompt exam. Mention rash, eye redness, fever, marked tiredness, or trouble making a fist in the morning.

Soreness that keeps worsening at rest or wakes you nightly also needs review. Tell the doctor about weight loss, night sweats, prior cancer, or a new lump.

## What to do after medicine, travel, or illness

Call the doctor who ordered your medicine when aches begin after starting or changing it. Don’t stop an important prescription suddenly unless that doctor tells you to do so.

Tell your doctor about recent travel, bites, fever, rash, or stomach illness. Dates matter because they show whether the illness came before the joint soreness.

A EULAR task force of 18 rheumatologists, 3 health professionals, 2 patients and a methodologist defined which joint symptoms, in the absence of any visible swelling, should make a clinician suspect that inflammatory arthritis is coming. Seven parameters survived: symptom duration under one year, symptoms in the knuckle (metacarpophalangeal) joints, morning stiffness lasting 60 minutes or more, symptoms worst in the early morning, a first-degree relative with rheumatoid arthritis, difficulty making a fist, and a positive squeeze test of the knuckles. The combination identified at-risk patients with an area under the ROC curve of 0.92 (95% CI 0.87-0.96).

Applying broader inclusion criteria, the overall symptomatic rate was 75% (63-84%) and the CHRONICITY rate - symptoms persisting beyond the acute phase - was 44% (31-57%). Mortality was 0.3%.
